Equine Mortality Application
- Required for all policies. (Be sure application is fully completed, signed and dated.)

Veterinary Certificate of Examination
To be filled out by your Veterinarian.
- A current examination is required to be completed by your veterinarian for horses valued in excess of $50,000; foals under 60 days of age; all horses used for racing; and may be required for horses with prior health conditions including previous surgery, lameness, illness or injury. Mortality insurance on senior horses may also require a current vet examination. The veterinary report cannot be more than 30 days old at the time your policy begins.

Statement of Health
- The insurance company will accept a health statement completed by you on horses to be insured at $50,000 or less and foals over 60 days of age. The health statement will ask specific questions regarding the health history of the horse to be insured. This report will save you the expense of an insurance veterinarian examination.

Justification of Value Forms
- The amount of insurance you request for your horse must be justified. The purchase price is the amount accepted for recent purchases. Training, showing, racing, and breeding information are used to increase the amount of insurance over your purchase price. The amount of insurance on homebred horses is usually based on the stud fee paid to produce the foal, but sales of similar offspring are taken into consideration as is the performance record of siblings. A certified appraisal is also considered in establishing the market value of your horse.

Care Custudy And Control Application 
This is legal liability coverage for horses entrusted to you by their owners. Consider this coverage if you board, train, or breed horses for others. If one of these horses is injured or dies while in your care and you are found negligent, this policy will provide coverage for the medical care or replacement cost of the horse up to the coverage limits. Defense costs are also covered. Various policy limits are available on a per horse/per year basis.
If you trailer non-owned horses, this is essential insurance. If you are transporting a horse owned by someone else and are involved in a vehicular accident deemed to be your fault, you could be held legally responsible for the injury or death to the horse you are hauling.

Race Horse Owner's and Trainer's Commercial General Liability
If you own or train racehorses, this policy will provide liability insurance protection for bodily injury or property damage to others caused by your equine activities.
The policy can include care, custody and control liability for non-owned horses in your care including during transportation from one racetrack to another.
